Neuropathische pijn bij Parkinson wordt sterk onderschat, maar komt veel voor. Zeker is er in onze medische wereld veel te weinig aandacht voor neuropathische pijnen. De ziekte van Parkinson wordt van oudsher gezien als een ziekte met alleen motorische stoornissen, maar al heel lang is bekend dat er veel meer symptomen bestaan die de patienten kunnen hinderen. Bijvoorbeeld geheugenstoornissen en depressies. Maar dus ook neuropathische pijnen. Op het internationale congres over de ziekte van Parkinson in Parijs, 2009 vonden we het volgende:

Neuropathische pijn bij Parkinson 

Gebruik makend van de DN4 schaal om neuropathische pijn te vinden vonden Franse onderzoekers bij een onderzoek onder 135 Parkinson-patienten het volgende:

135 PD patients were included. 71% of the PD patients were painful. Among these painful patients, 66% had PD related pain. Among PD related pain, 70% of the patients had neuropathic pain. 

En de conclusie was:

Prevalence of chronic pain, especially neuropathic pain is high in PD. This is in accordance with previous studies that have shown lower objective and subjective pain thresholds in Parkinson’s disease.  

Parkinson: vaak pijn 

Een andere onderzoeksgroep vond ook dat pijn vaak voorkomt bij de ziekte van Parkinson:

44 % of the patients considered pain to be caused by PD. The average intensity of pain on VAS was 6 on a scale from 0 (no pain) to 10 (worst pain). The intensity of pain was correlated with higher scores on PDQ-39, indicating that quality of life in PD patients is much affected by pain. Patients, who experienced pain, had also more severe depressive symptoms. No correlation between disease staging and intensity of pain was observed. 

Zij gaven ook aan dat dit een sterk negatief effect heeft op de kwaliteit van leven. 

Parkinson en pijndrempel 

Tenslotte is de pijndrempel bij patienten met de ziekte van Parkinson veranderd, en verminderd. 

Pain threshold and pain tolerance were significantly lower in PD patients for both right and left hand and foot than in normal subjects. In PD patients with pain, pain threshold and tolerance obtained on the painful limb (regardless of muscular or neuropathic pain) were lower compared to those obtained on the non painful limb and in pain-free PD patients. 

Samenvattend: Langzaam dringt het besef door dat Parkinson-patienten vaak last hebben van neuropathische pijn. Er is een grote onderherkenning en onderbehandeling. Bij ons instituut druppelen ook Parkinson-patienten binnen met neuropathische pijnen, maar dat is sporadisch. Het lijkt erop dat Parkinson-patienten berusten in het hebben van pijn. Dat is niet nodig! De pijn kan meestal goed behandeld worden, en de kwaliteit van leven wordt daardoor aanzienlijk beter!
